All about me

Like all individuals, people diagnosed with dementia are whole people with their own values, preferences and personal life stories.

All about me is a resource for people with dementia to tell health-care providers about themselves –their needs, likes, dislikes and interests.

Knowing more about the person with dementia helps community support workers and staff in long-term care homes build relationships and support personalized care.
